- It would be useful to have the applicant’s NHS number but you can also complete without it.

- This form could take between 5 and 20 minutes to complete depending on the complexity of your answers.

Here at Sefton & Liverpool CAMHS we work with children and young people to help you make sense of any mental health and emotional wellbeing challenges you might be facing.
