The personal information we collect about you when you make a referral may include but is not limited to the following information about you:
Your name (including your first name(s) and surname);

Your postal address; and
Your phone number (landline and/or mobile)
Your date of birth
Your education / employment information
We may also collect other details about you that you or others provide to us – such as Equality data.
If you are contacting us in a professional capacity, we may also collect the following information:
Organisation/place of work details
Business contact details
When you use our website we will collect:
Your IP address, and details of web browser and which version of it you used information on how you use our website, using cookies, that will help us improve the website details of transactions you carry out through our website and of the fulfilment of your orders.
How we will use this information:
Your personal information will only be used by us to:
Send to the correct partner agency so they can action your referral;
Aggregated and anonymous data is provided to funders of the service to meet our obligations to commissioners
Maintain our organisational records.
Sharing your personal information:
The Sefton & Liverpool CAMHS partnership will not share your personal details with third parties, except where companies / organisations are providing services on our behalf, in these cases your data may be shared with partners who commission our services.
The Data Protection Act
The Data Protection Act requires all the information is kept safely and also gives all patients certain rights.
Patients, or their guardians, have the right to see the information which we keep.
All information is kept securely and only those who need it to help with treatment, have access to it.
We only pass on the information about our patients if the other person has a genuine need to know or to protect your health.
We only pass on the information which is needed and no more.
You have the right to ask for us to correct incorrect data
Each type of record is held for a certain amount of time before it is recognised as no longer being needed and can be confidentially destroyed. National time periods are shown in the Records Management Code of Practice for Health and Social Care 2016’
You have the right to confidentiality under the General Data Protection Regulations 2018, the Human Rights Act 1998 and the common law duty of confidence (the Disability Discrimination and the Race Relations Acts may also apply).

As this platform is a partnership of providers (Alder Hey, YPAS, MHST, Venus, Parenting 2000, Liverpool CAMHS partnership, MHA). These organistions are required to work to the same standards of confidentiality as all NHS employees, and we have agreements and contracts in place to ensure these standards are maintained.
Security – How we look after and safeguard your data:
We will take reasonable precautions to prevent the loss, misuse or unauthorised alteration of information you give us.
We use appropriate technical and organisational measures such as storing your information on secure servers, encrypting transfers of data to or from our servers using Secure Sockets Layer (SSL) technology, only granting access to your information where necessary.
Communications in connection with the website and its content may be sent to you by us by e-mail. For ease of use and compatibility, communications will not be sent to you in an encrypted form.
E-mail is not a fully secure means of communication. Whilst we try to keep our systems and communications protected against viruses and other harmful effects we cannot guarantee this.
How long do we store your data for:
We will retain your data for no longer than necessary, taking into account any training obligations we have and any other legal basis we have for using your information
